This includes a comprehensive evaluation of the senior's needs and key issues followed by a plan of care identifying service options and recommendations.
ONGOING CARE MANAGEMENT
This is an ongoing service tailored to the senior's needs to help monitor and manage all aspects of their care. We will serve as a constant source of support and guidance for the senior and their family as they navigate the healthcare system.
